Please either fill in the pdf and save it or print out and fill in by hand (and re-scan it).

Step Two: Send it off

IF YOU’VE USED THE ONLINE FORM, then relax (but make sure you’ve paid the fee)

or email the pdf form to the email detailed on the form.

Step Three: Pay the Fee
