Please either fill in the pdf and save it or print out and fill in by hand (and re-scan it).

Step Two: Send it off

IF YOU’VE USED THE ONLINE FORM, then relax (but make sure you’ve paid the fee)

or email the completed form (email address is detailed on the form).

Step Three: Pay the Fee

Please send £10 by Bank Transfer or

a PayPal payment of £10.61 as a business (to cover PayPal fees)
